CLASS ENROLLMENT INFORMATION

1. Decide which class you would like to attend by checking the current Class Schedule for upcoming start dates.

2. Call 512.551.0044 x102 or email us to confirm that there are spots available.

3. Download and fill out the Enrollment Form. The enrollment form is a pdf form – you can fill out and print the form on your computer if you have the latest version of Acrobat Reader, a free program available from Adobe. (Download Adobe Reader Here) You can email, fax or mail your form to the address at the top of the form.

4. Mail your payment to the address at the top of the Enrollment Form or drop your payment off at Mud Puppies. Please make check payable to:   Sublime Canines
We can also accept credit cards through PayPal.  A $5.00 fee will be added if paying through PayPal.  Please let us know if you would like us to invoice you through PayPal.
**Your Enrollment Form AND payment must be received to reserve your spot in class**

5. Please provide us with a copy of your dog’s current vaccination records from your vet.  You can email or mail these to us with your enrollment form, have your vet fax them to us or bring them with you to the first class. We require that your dog be current on the following vaccinations:

Rabies
DHLPP (Distemper/Parvo)
Bordetella (within the last 6 months)

6. Once your Enrollment Form and payment have been received you will receive an email from us confirming your enrollment in class.
